Customers who purchase O’Pet Curv Wireless ECG Sensor, Super Air Smart ICU during the promotional period dates (1/1/26-3/31/26) will receive the promotional items as below.

1. Super Air Smart ICU

From January 1 through March 31, 2026, customers in the U.S. who purchase any twin models of the Super Air Smart ICU (Air-C1-T, Air-C2-T, Air-C4-T, or Air-C4-C) during the promotional period will receive Free “Saver” Delivery.

How To Get a promotional item from Bionet America, Inc.

  1. Complete ALL information – including serial number for each product. Incomplete and/or illegible forms will not be processed.
  2. Include the invoice or sales slip (photocopy acceptable) which shows the model number and date of purchase.
  3. List the model name/number and serial number on this sheet.
  4. This Claim Form must be submitted no later than 04/30/2026.
  5. Retain a copy of submitted materials for your records.
  6. All purchases must be made through an authorized Bionet distributor or Bionet.

 

  • All red fields, and those with an asterisk, are required for a successful submission. 
    Your browser will alert you when you have successfully submitted your request. You will also receive an email notification, immediately after submission.
