2. Include the invoice or sales slip (photocopy acceptable) which shows the model number and date of purchase.
3. Select model names on this sheet.
4. Retain a copy of submitted materials for your records.
5. All purchases must be made through an authorized Bionet distributor.
6. Rebate applies to purchase price only and excludes any taxes, fees, or shipping costs
7. This promotion does not apply to any customers outside of the US.
